While you don't need an attorney to complete the document, it is considered binding once it bears witness signatures or notarization. For your family, Five Wishes ends the bedside guessing game of what you want or don't want at the end of your life. In simple language, it walks you through the decision-making process on topics such as designating a decision-maker in the event you are non-responsive, your preferred level of life-sustaining medical support, and your desired alertness when pain medication is needed.
